You’ll experience the same level of safety and care that you would at our Medical Center, and where you are most comfortable — right at home. The LRH at Home care team closely monitors your condition and makes any treatment changes needed for optimal recovery!

How does Lakeland Regional Health at Home work?

Our goal is to get you better as soon as possible, with safety as our top priority. Lakeland Regional Health at Home has countless protocols and a full team in place to ensure you get the attention and care needed to recover. Lakeland Regional Health at Home is covered by Medicare at no additional cost to the patient — it is considered the same as an inpatient stay in the hospital.

Remote Monitoring

Upon admission to the program, you will receive a remote monitoring device that allows the LRH at Home team to continuously obtain and deliver your vital signs.

Home Visits

A physician or nurse will visit your home at least twice daily to support medical and recovery needs, provide monitoring and education, and help coordinate any virtual visits as needed.

24/7 Access

When receiving care, you will have 24/7 access to the LRH at Home care team if you have questions, concerns, or if a new health issue arises.

Frequently Asked Questions

Now that you are home, your hospital-level care will continue. You will receive regular in-person visits with the care team, as well as virtual check-ins. Your medical condition will be monitored continuously, and the LRH at Home team is available 24/7 for any questions or concerns.

A dedicated care team member will visit your home at least twice a day to check on your progress, administer necessary treatments, and answer questions.

You can reach the LRH at Home team by calling 863.284.1950. For non-urgent questions, you can also message from the LRH iPad. For urgent medical issues, call 911 immediately.

To ensure a smooth recovery, follow your care plan as instructed. Keep your monitoring devices on and charged to allow for real-time tracking, report symptoms or concerns immediately, take medications as prescribed, keep a clean recovery space, and follow drinking and eating instructions.

Discharge from LRH at Home happens once your condition stabilizes and hospital-level care is no longer needed. You may at that time transition to home health service, outpatient follow-up, or primary care.

The team will ensure medications and necessary equipment are delivered daily. They will also coordinate any needed services such as labs, imaging tests, or therapy.
