Self-Enroll

If you have been a patient of Lakeland Regional Health Medical Center or one of our hospital-based services (Wound Care Center, Infusion Center, Rehabilitation Services) you can self-enroll. You can access the information by visiting the website specified in the instructions or by scanning the provided QR code.

Mobile App

Download the Cerner Health Patient Portal (HealtheLife) from your mobile App Store. Once installed, search for Lakeland Regional Health and follow the prompts to sign up.

By Invitation

At Your Appointment Check-In

The easiest way to sign up is to provide your email address to the registration staff when you are getting checked in. You will then receive an invitation sent by Lakeland Regional Health (noreply@iqhealth.com). Simply click on the link in your email and provide the information requested to complete the registration process.
