Physician Group Portal FAQs

FollowMyHealth® (FMH) is the Lakeland Regional Health Physician Group patient portal for your doctor's office visits. The portal is a secure online service that provides you with access to your healthcare information.
With FollowMyHealth®, you can:
  • Review your medical information online in a safe, secure environment
  • Communicate privately with physicians via secure messaging
  • View test and lab results from your doctor
  • View a summary of your office visit
  • Request prescription refills
  • Schedule or change appointments
  • View and pay bills
The hospital will continue to use the Relay Health portal, so please continue to access this portal for any hospital related care. Nothing will change with your Relay Health log in information for hospital services.
The LRH Physician Group is consolidating its portals into one with FollowMyHealth®. You will now be able to view your doctor's office medical information and billing information all in FollowMyHealth®.
Your health record is portable and travels with you. You can access your FollowMyHealth® Patient Portal account 24 hours a day, seven days a week, via computer, tablet or smart phone. FollowMyHealth® is supported by modern versions of Google Chrome, Mozilla Firefox, and Safari, and Internet Explorer 8 and higher.
Yes, if you have an Apple or Android based device, you can download the app from the iTunes App Store or the Google Play marketplace store. It is easier to first establish your account on a computer. The name of the app is FollowMyHealth.
No. FollowMyHealth® Patient Portal is offered free of charge to our patients.

Once a Registration Team Member has confirmed your identity, an invitation will be sent to the email that your provided. The email will be sent from noreply@followmyhealth.com. Please click on the link in your email and provide the information requested to complete the registration process.
Your invitation code will be the last four digits of your Social Security number.
No. Each user should provide a unique email address.
Click on the My Info Tab. In Demographics - General, locate contact information. Click Change next to Email.
Visit http://myLRH.org/patient-portal and click on the link that says LRH Physician Group. You will be redirected to https://mylrh.followmyhealth.com. Click on the Sign In button and select the Forgot your password link. Enter your username and select reset. If the username was correct, an email with instructions will be sent to the email address on file.
There are two options to recover your username.
Option #1: When you created your account, you would have received an email confirming your specific FMH Secure username. Please go to your email and do a search for an email with the following information:
- it is coming fromnoreply@followmyhealth.com
- the subject of the email is "FMH Secure Login Account Created"
- if you have this email it should have your correct, connected username in it
Option #2: Contact Lakeland Regional Health at 863.284.5000 for assistance with recovering your FMH Secure username. In going this route, you will need to confirm some personal information including your name, date of birth, and email address. In an effort to protect your privacy, this method of retrieval also requires additional support personnel and will require a return phone call to you.
All the information you provide during the online registration process is confidentially processed. Once your registration is complete, you will be able to safely view and print your medical information for personal use or to share with your other doctors and healthcare providers.
Although Lakeland Regional Health Patient Portal uses safeguards, there are other safety tips you should follow when accessing this service. Always remember to protect your username and password from others and make sure to only log on from a personal or secure computer or mobile device.
Once you complete the registration process, you are the only one that can see your health information.
The only information updates that are currently sent to your doctor are changes made under the My Info section. This includes patient demographics, insurance, and preferred pharmacy only if you elect to Send this information to LRH. Any changes to your personal health information are for your personal record keeping, so you will need to inform your doctor of this information during your next appointment.
You can make this selection once you are logged into your portal account. Click My Account and select Preferences from the drop down. On the screen under the Account Preferences section there is a button to Delete your UHR (Universal Health Record). Click Yes in the warning box to permanently delete your UHR.
The minimum age for creating an account is 18 years. Lakeland Regional's current policy does not allow a parent or guardian to create a separate account for a minor child.
At this time, Lakeland Regional Health's current policy does not allow proxy access. To protect the privacy of all LRH patients, please do not send any information from your FMH account regarding another patient.
Yes. You can make this selection once you are logged in to your portal account. From the Home Page, the language option is available in the upper right hand corner of the screen. Click on the drop down under English and select Español
Yes. Navigate to your My Health section. In the top right corner of each section are buttons to Email, Print, and Fax. This will perform the selection action on the section of the record you are in. You can also print individual items if you open the item first. On the Summary screen, there are also selections for Email All and Print All. Please note that your devices may not be set up for printing or faxing.
Log into FollowMyHealth®. Go to "My Account," then go to "My Connections." The providers listed are LRH providers that you have seen or that you have an appointment with in the future.
FMH offers a wide variety of support from within the portal. From the Home Screen, select My Account and from the drop down select Support. Once the Support Information opens, you can click Knowledge Base for Frequently Asked Questions or click the User Manual for step-by-step instructions on using the portal.
