WHAT IS THE CONGREGATIONAL HEALTH PARTNERSHIP?

The Congregational Health Partnership supports congregations as they seek to improve the health of their members. The Partnership is formed when a congregation selects one or more Community Health Liaisons who will work with the Lakeland Regional Health Congregational Health Program Manager to understand the specific needs of the congregation, provide links to community and health resources, and establish programs that make measurable improvements in the health of individual members and the congregation as a whole.

WHY SHOULD CONGREGATIONS HAVE COMMUNITY HEALTH LIAISONS?

Understanding health information and finding healthcare resources can be challenging for some individuals. Community Health Liaisons can help make healthcare more understandable and accessible to congregants by serving as trusted friends and guides on the path to better health.

ROLE OF THE COMMUNITY HEALTH LIAISON

The Community Health Liaison works closely with Lakeland Regional Health to identify education opportunities. They promote healthy lifestyle habits, monitor health-related needs among the congregation and help members understand what steps can be taken to become or remain as healthy as possible.

ROLE OF CONGREGATIONAL LEADERSHIP

Congregational leadership will provide continuous support and promotion of both the program and
community activities that assist in improving access to healthcare.
