How do I get a copy of my medical record(s)?

You will need to contact the Medical Records department at 863.687.1067. Because of confidentiality regulations, these requests cannot be made online.

Please note that State and Federal laws permit fees to be charged for copies of medical records that are not requested by health care providers for direct patient care.  To reduce the cost, patients should consider requesting specific information rather than a complete record.

In addition, LRH offers patients free access to the LRH Patient Portal, which provides secure online access to their healthcare information. It also allows a safe and easy way to share electronic medical information with healthcare facilities and providers.  The LRH Patient Portal can be accessed at

How do I register for childbirth classes?

How do I obtain a birth certificate?

To obtain a birth certificate for someone born at Lakeland Regional Health Medical Center, contact the Polk County Office of Vital Statistics at 863.519.7900.
